- Your name, phone number and email address when you book an appointment, fill out a form or contact us.
- Appointment details, such as the office, date and time you choose, and any answers you give on our booking form.
- Anything you include in a message to us.
Information collected automatically
- Basic technical information, such as your browser type, device, IP address and the pages you visit.
- Cookies and similar technologies used by our website and by the booking, form and map tools embedded on it. You can block or delete cookies in your browser settings; some features, like online booking, may not work without them.

- To schedule, confirm and remind you about appointments.
- To respond to your questions and requests.

Text messages (SMS)
When you book an appointment or send us your phone number and agree to receive texts, you consent to receive text messages from Opensky Podiatry about your appointments and inquiries.
- What you'll receive: appointment confirmations, reminders, rescheduling and cancellation notices, and replies to questions you send us.
- How often: message frequency varies, usually a few messages per appointment.
- Cost: message and data rates may apply, depending on your mobile plan.
- To stop: reply STOP to any message to opt out. You'll get one message confirming you've been unsubscribed, and we won't text you again unless you opt back in.
- For help: reply HELP, or call us at 708-747-8994.
- Consent to receive text messages is not a condition of receiving care or making a purchase.
- Carriers are not liable for delayed or undelivered messages.
We do not sell, rent or share your mobile phone number or text messaging consent with third parties or affiliates for their marketing or promotional purposes. We share mobile information only with service providers that help us send messages on our behalf, and they may use it only for that purpose.
Your health information
As a healthcare provider, we protect your medical information as required by the Health Insurance Portability and Accountability Act (HIPAA).
